Terms and conditions for Maternity Leave Membership

FIT4MOM PREPAID PLAN AGREEMENT

This contract is made between the client and Fit4Mom East DuPage West Cook (Pawelko Dash LLC) and is a prepaid plan agreement and may not be terminated by either party. This membership allows you to attend unlimited drop-in classes over a 7 week period.

AUTHORIZATION FOR PAYMENT

I authorize Fit4Mom East DuPage West Cook to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until my membership through the Fit4Mom East DuPage West Cook has completed.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Fit4Mom to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started for the first month and then on

MEMBERSHIP UPGRADE POLICY

Memberships may be updated to a monthly plan at the completion of your 7 week prepaid plan if you so choose. This plan will be valued as one month of membership to meet the three month minimum.

INITIAL 10-DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM East DuPage West Cook within the initial ten days. You are responsible for paying for the classes already attended.

CLASS REGISTRATION AND CANCELLATION POLICY

Please register if you plan to attend class and cancel your registration up to 1 hour before class. Cancellations less than 1 hour before class and no shows may be charged a late cancellation fee of up to $10.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
